Balkans are probably from the Balkan Peninsula, hence their name.
The Elite Crew have no actual IRL counterpart aswell.
Phoenix Connexion, fictional, once again.
Professionals, also fictional. Kind of, they're bank robbers.
Separatists are the only non-fictional organization, based on the spanish ETA which was disbanded in 2011.
I thought it was like this:
Anarchist: What you said
Balkan: Criminals from the balkans area
1337 Krew: ISIS/ al-queda
Phoenix Connection: IRA
Professional: Bank Robbers
Ah, yes, true, I forgot about the Phoenix Connexion's inspiration. I doubt the Elite Crew is inspired by ISIS however, because it only became popular in 2014 when it separated from al-Qaeda, and besides, neither of the faction's militants dress like that. Valve probably avoided using those factions as inspiration because of their image in popular culture as EVIL SATAN WORSHIPPERS or whatever, and many would revolt to the sight of a game where you play as an ISIS fighter.

That's probably it. Frankly, they could be someone's neighbours that got tired of little Johnny's dog and decided to do some vigilante work by bombing up strategic locations in order to scare the dog into running far away. Valve never said anything about it.
